Consumer Health Data Privacy Policy

This Consumer Health Data Privacy Policy describes how Align & Refine Massage LLC (“Align & Refine,” “I,” “me,” or “my”) collects, uses, and shares consumer health data and explains the rights available to consumers under applicable Washington law.This policy applies specifically to consumer health data and is separate from Align & Refine's general Privacy Policy.

Consumer Health Data I May Collect

Depending on your interactions with Align & Refine and the services you request, I may collect consumer health data such as:Areas of pain, discomfort, soreness, or muscular tensionInjuries and relevant health historyPhysical conditions or limitationsMobility or range-of-motion concernsSymptoms relevant to massage therapyInformation about previous treatment or care when relevantMassage goals, preferences, and areas you would like addressed or avoidedInformation about medications or medical conditions when relevant to the safe provision of massage therapyInformation concerning massage services requested or receivedOther health information you voluntarily provide in an intake form, communication, or conversationInformation identifying you as someone seeking or receiving massage therapyYour service location when necessary to provide requested in-home massage servicesI intend to collect only information reasonably necessary to safely provide and administer the massage services you request.

Why Consumer Health Data Is Collected

Consumer health data may be collected and used to: Determine whether massage therapy can be provided safely and appropriately. Understand your goals and preferences. Personalize your massage session. Identify areas requiring attention or areas that should be avoided. Communicate with you regarding your care. Provide and document massage therapy services. Maintain records required for professional, business, or legal purposes. Respond to requests concerning your massage services. Washington law generally limits collection of consumer health data to collection with consent or collection necessary to provide a product or service the consumer requested.. Sources of Consumer Health Data. Consumer health data may be collected directly from you through: Client intake and health-history forms. Informed-consent forms. Appointment or session requests. Email or other direct communications. Conversations before, during, or after a massage session. Information you voluntarily provide while receiving services. 

Sharing of Consumer Health Data

Align & Refine does not sell consumer health data. Consumer health data may be shared only when reasonably necessary to provide services you have requested, when you have provided any consent required by law, or when otherwise permitted or required by law. Depending on the systems used by Align & Refine, information may be processed by service providers that support functions such as secure form collection, record storage, communications, or other business operations.

Your Consumer Health Data Rights

Subject to applicable law, you may have the right to: Confirm whether Align & Refine is collecting, sharing, or selling your consumer health data. Access consumer health data concerning you. Receive information about third parties or affiliates with whom your consumer health data has been shared or sold. Withdraw consent for future collection or sharing when collection or sharing is based on consent. Request deletion of consumer health data concerning you. Requests may be submitted using the contact information below. Certain records may be subject to professional or legal record-retention requirements, which may affect how a request can be fulfilled. Washington's MHMDA provides consumers rights concerning access, withdrawal of consent, and deletion of covered consumer health data. 

Security

Align & Refine uses reasonable administrative, technical, and physical safeguards appropriate to the nature and sensitivity of consumer health data. Access to consumer health information is limited to what is reasonably necessary to provide services and operate the practice.

Changes to This Policy

If the categories of consumer health data collected, the purposes for which the information is used, or applicable data-sharing practices materially change, this policy will be updated as required. Where applicable, affirmative consent will be obtained before collecting, using, or sharing additional categories of consumer health data or using existing consumer health data for additional purposes not disclosed in this policy. Washington law specifically addresses those circumstances.
